58-Year-Old Man Dies and 3 Hospitalized after a Fatal Two-Vehicle Collision near Olney Avenue

PHOENIX, AZ (March 2, 2023) – Wednesday afternoon, a two-car collision near 19th Avenue left Forrest Wade dead and three others injured.

The incident happened on February 22nd, near 19th Avenue and Olney Avenue, just south of Dobbins Road.

According to reports, two vehicles collided near the intersection.

Following the collision, officers found Wade unconscious and three others injured. Medics transported all four victims to a local hospital. There, officials 58-year-old Forrest Wade dead, due to the extent of his condition. The extent of the other victims’ injuries is currently unknown.

Meanwhile, police said that speed might be the factor of the incident. Charges may be cited once the investigation is final.

At this time, the police have not yet determined what caused the incident and further investigation is still ongoing.
